A failed pipeline reports that an operation did not finish successfully. It does not establish which earlier actions finished, whether an external request took effect or whether repeating the job is safe. A retry decision record gives the receiving engineer a bounded set of facts to inspect before execution resumes.
Record the operation, not only the error message
Start with the candidate artifact, configuration reference, target environment and job identity. Identify the last action confirmed by the target system, not just the last line printed by the runner. A network timeout may hide a successful remote operation; a permission denial may occur after an earlier step changed a resource.
Keep observation and interpretation separate. Write that a resource exists only when the authorized check established it. If its state cannot be read with the available access, mark the fact unknown and name the owner who can inspect it. Do not convert missing visibility into a claim that nothing changed.
Classify what a repeated action could do
An operation designed to be idempotent should converge on the intended result when repeated under its stated assumptions. That property needs evidence for the particular action and inputs; it is not supplied by calling the entire workflow a deployment.
Inspect provisioning, data mutation, notification and integration steps separately. A repeatable configuration step may sit next to a one-time data change or an external event. Identify any duplicate side effect and the evidence needed to avoid it. Do not run a destructive operation simply to make a tool's state look consistent.
Define three explicit decisions
The first decision is stop and inspect: execution remains paused while the authorized owner resolves unknown state. The second is a bounded retry: the owner has evidence that the specific action can be repeated with the same accepted inputs. The third is recovery or forward correction: the prior state or dependencies require another reviewed change rather than another attempt at the old job.
